Consultations

The first step to begin treatment is to schedule an initial evaluation with Dr. Kondrot. This evaluation can be scheduled by calling our office at 1-800-430-9328 or by filling out the online form.

During this evaluation Dr. Kondrot will review your medical records and discuss the possible alternative treatments that are recommended to treat your condition.

This is also the best way to determine if  you are a candidate for the 3 day program. If you are not able to come to the office in Florida  we can schedule a telephone/ Skype  consultation,  but we will need a copy of your last eye records and photographs if they are available.

Please print and complete the following forms prior to your scheduled consultation. Once we receive the online form and payment we will schedule your evaluation within one week

• Last eye exam
• Copies of Eye Photographs (if available)

Fees

Initial Consultation $500.00.
If you are a candidate for the 3 day program this fee will then be applied.
If you are prescribed a homeopathic remedy this will include a one month follow up
